Supplemental Application

Supplemental Application Packets are to be completed simultaneously with your PharmCAS application online (www.pharmcas.org), please see the School of Pharmacy mailing address below to submit your Supplemental Application Packets." A complete Supplemental Application Packet consists of a Supplemental Application; a non-refundable Application Fee of $25.00 (Check or Money Order made out to the University of Maryland Eastern Shore); a Biographical Sketch (a one page summary of who you are, what matters most to you, and how you would add value to the School of Pharmacy); a 2X2 Passport Photo; and, the Maryland Residency Form (if applicable).  Only completed Supplemental Application Packets postmarked on or before March 1, 2010, along with the corresponding PharmCAS Application will be processed and evaluated for interview consideration.  Supplemental Application Packets should be mailed to the following address below:
