Supplemental
Application Packets are to be completed simultaneously
with your PharmCAS application online (www.pharmcas.org). A
complete Supplemental Application consists of Supplemental Application (click here); a
non-refundable Application Fee of $40.00 (Check or Money Order made out to the
University of Maryland Eastern Shore); a Biographical Sketch (a one page
summary of who you are, what matters most to you, and how you would add value
to the School of Pharmacy); a 2X2 Passport Size Photo; and, the Maryland
Residency Form (if applicable). Only completed Supplemental Application
submitted on or before February 1, 2013, along with the corresponding PharmCAS
Application will be processed and evaluated for interview consideration. Please
note that the supplemental application is to be submitted electronically.
The supplemental application fee and passport size photo should be mailed to
the following address below:

Please note that you
should save the application to your computer before
completing it. If you have difficulty
submitting your application electronically, send it as an attachment to rxstudentaffairs@umes.edu. When your application has been received, you
will receive an e-mail confirmation. The
application will not submit properly on a MAC computer. MAC users may need to print the application and
fax it to 410-651-8394.
Applicants are responsible for notifying the Office of Student
Affairs (Admissions) of any changes in their mailing address, phone number,
and/or e-mail address.
